Field Trip Registration Form

  • (Field trips generally last 2 hours, depending on age, with additional time for lunch or craft)
  • $5.00 per student/parent (chaperone)/sibling
    $50 non-refundable deposit required to book field trip
  • ONE teacher and ONE aide are required for every 30 students

  • Please collect money from all attendees and make 1 payment upon arrival.
  • ALL participants must complete a liability waiver – due upon arrival.
